Actually, if done right, in theory everybody could win.  There are
real economic advantages (many billions of dollars worth) to forcing
everybody into the pool.  And the big insurance companies are
currently altogether too close to nationalization to pushback too
hard.  (They've actually done pretty well on secondary Medicare
insurance.)

As usual, in practice that are problems that will certainly result in
winners and losers.  But the winners are becoming more numerous as
companies cut back on coverage for even fulltime workers, and people
who don't want to dedicate their lives to the company become a larger
share of the workforce, and often have no employer coverage at all,
and thus end up facing penalty rates due to adverse selection and
transactions costs.
