Hey, nothing to it.  I've been stuck on problems like this
myself.  It can be really frustrating until to finally figure out
where the problem lies.  We WILL solve this one.

This could well be it.  You say that you start up cardmgr from
within an initial ramdisk.  Mount that disk image with loopback,
and look for a file called etc/pcmcia/config.opts.  This tells
the PCMCIA subsystem which bits of memory it can use, which it
can't, and which IRQ's are off-limits.  If there is already an
entry that says "exclude irq 5", then we're back to the drawing
board.  But if there isn't one, it's worth a try.  Add that line
to this file.  Unmount the disk image, and try booting again.

Let me know how things turn out.  I may have run out of ideas on
this one, but if it doesn't work, let me know anyway, and we'll
see if another strategy emerges between us.  Can't promise
anything, but two heads are often better than one.
