Just my 2 yen on this:

I have been using Asahi-net for the last 8-9 years in at least 8
different locations (private and business) from the ISDN days.
They simply rock, not to mention they have English support. Most of
the time I have servers with static IPs on B-flets (the name changed
many times, but it is a 100Mbps FTTH).
I routinely get 5-8MB/s from kernel.org and sf.net mirrors for
download, upload is about the same.

If you apply through Asahi-net you won't have to deal with NTT
directly and get one bill. The only downturn for some specific cases
is that Asahi-net ONLY accepts credit cards.

I know quite a few people using the service, I think TLUG have (had?)
some sponsorship from them.

If you want full control and have 24/7 running box, get a ONU (Optical
Network Unit) plug the RJ45 end to your box and run your own
router/firewall/DNS cache on it (ppp/iptables/dnscache).

Unless you really want to be cheap get dedicated data FTTH and avoid
the cableTV.
