The pre-paid *debit* Mastercard cards I've seen are just sold
over-the-counter. No address needed, not even a name.

Aren't they linked to the country where they are sold?  I am quite sure they are.
For instance, cards emitted in France have different costs, depending you use it
locally or abroad. Also they include some features which are specific.
For instance travel insurance (which also means clearly that they are linked to
France).

Also, they are not really sold "over the counter": They are usually not linked a a bank
account. However, they are linked to a web site (the provider) so you can recharge
it, block it, etc...

To come back to Amazon, they will accept prepaid cards, but during registration you
must put a "credit/debit card on file (for verification of the country you live in)...", according
