In Saitama, according to the Saitama Police web site [1], you will need
both a jyuminhyo and your residence card [the new Alien Registration
Certificate]. 

Other requirements include two pictures, your old and new passports to
confirm you were residing in a foreign country for at least three
months, and an "official" translation of your foreign driver's license
prepared by JAF.  

[When my son switched from his USA license, we prepared our own
translation, but it was rejected (it was incomplete, missing some hidden
code number) :-/ ]

Anyway, apparently this will all be explained to you in Japanese at your
preliminary review, which is required before making an application.
Don't speak Japanese? - bring an interpreter.

IIRC, in my son's case, he did have to take a road test (passed on
second try).
