I hate to add that I have also experienced "lost" files on a USB device that I believed to have cleanly dismounted before removing.  This hasn't happened frequently enough for me to spend time trouleshooting, and I've never lost anything permanently.

 

This was always with a flash memory device rather than a physical drive.

 

In all cases I've reused the same device succesfully, and thus assume the issue to be somewhere in the timing of the writes or dismount, rather than a failure of the USB device itself.
