SOLVED

As of mutt 1.5.5 or so, I think, the set alternates="scott@example.com" no
longer worked, but was considered an unknown variable.  I commented it
out and never fixed it.  

Using the new syntax
alternates "scott@example.com|scott@example.com" solved the problem

I ~knew~ the answer had to be there somewhere.
