As mentioned, you get an MNP request form from your current carrier. rI've gone from AU to Softbank, and then from SB to an MVNO keeping the same number.
If you use LINE, you should look up how to transfer your history before you make the leap.

Q2: Recommendations for the most effective/cheapest plans? 
 
I went from SoftBank to Y!Mobile, because they offered me a coupon deal. If you can swing by the MVNO counters at Bic or Yodobashi, you can negotiate.
Also helps to know if you qualify for some kind of package through another contract - I have fibre through Softbank as well so I get a small discount monthly.

Y!Mobile currently has a double-the-data campaign (I get the 6GB plan for the price of 3GB). Check around the various counters, you know what  you want already, but comparing the various dealers in one place helped me a lot.
