"Is this a cat or a dog?" is what Bank of America uses to prevent
phishing.  (Actually, they allow you to choose one of several images;
if your image is not present or incorrect, you're being phished.)  I'd
already remembered that, but didn't hit on your excellent idea.
Perhaps combined with the list of options in an very mild captcha
format so that the multiple choice options can't be reliably read (ie,
it won't be checkboxes, you have to type in "dog" or "cat" or
"Tyrannosaurus Rex".)

Needs refinement, as my last tongue-over-teeth-but-not-quite-in-cheek
example indicates.

Once popular, simple arithmetic will only work for a while, because
the formalism is too easy to parse (and humans will be worse at doing
obfuscated arithmetic than computers are!)
