Andrew Schulman did that with DOS, Netware, and later Windows, and Matt Peitrick did that with windows before he started working at MS. For DOS there used to be a nifty program called "sourcer" from V communications. I know of at least one legit use of that tool where a programmer lost his source and used this tool to recover it rather than re-write it! I would presume tools in this area have advanced in the past 10 years or so.
