TLUG,
I got an off-list suggestion that works great:
> If that's the case, make sure you have xpdf-utils installed, and try: > pdfimages -f 1 -l 100 mydoc.pdf img
This command enabled me to extract the image as a ppm file, which opens in GIMP. From there I could manipulate the image and save into any format I wanted.
Thank you to everyone who made suggestions and provided advice.
