Yes, it needs to be patched, and you want to stay away from the qmail.org
stuff (at least, it never worked for me).

Get the source, patch, and instructions here:

Make sure you get the Japanese-patched Slang at the above URL as well,
and compile against it unless you know for sure that the Slang on
your system is a Japanese-patched one (it probably isn't, unless you 
installed it yourself), in which case you can compile against the one
you have installed, but you'll have to give the path to your Slang
to the configure script.

The current Japanese patch is against mutt-1.0pre3i.
