Every major teleco provides fiber service.  Even the Kansai Power
Company sells fiber service (presumably on their own fiber).  The crux,
however, is that every fiber provider utilizes NTT's equipment in one
way or another -- if for no other reason than the fact that they have to
for peering.  Even KDDI leases space in NTT's data centers, and they're
sworn enemies.

I've used OCN (NTT's own-branded ISP) fiber for the past 4 years, and
have only good things so say.  Their customer service was even quite
good when I was looking for help configuring my CTU to handle PPPoE for
multiple routers I have at my house.  "We don't officially support Cisco
equipment, but hold on a sec...", the tech said.  She got back to me in
less than 2 minutes with the answer.

I've heard stories of people getting reprimanded for more than 30GB of
bandwidth (down) per month.  Almost any consumer-marketed ISP will do
some port filtering to cut down on malicious traffic.  If you're worried
about this, then go with a business connection from a provider like IIJ.
 Be ready to pay out the nose, however.
