I don't have any special knowledge about this, but it's here (for
people who can read the 'panese):

It quite specifically says that it's about making sure people are in
"shakai hoken", and that from 2010-04-01 you'll have to show your
shakai hoken-sho at the window when you renew your visa. Wouldn't have
thought your Global Health Card or whatever you might have would cut
the mustard.

Doesn't say anything about pension contributions either way, but I'm
guessing that if you were applying as an individual you could get away
with just signing up for kokumin hoken, muttering something about
coming back tomorrow to do the pension and keeping your life savings
in something more likely to pay out than the Japanese pension system.
(Taliban war bonds, for example.)

Also it doesn't say anything about permanent residents, or about doing
the check when we next do our reentry permits or the new
Houmu-Kyoku-issued Gaijin Cards, but I suspect if they were planning
on doing that they'd have said so.
