not run across them if you started by calling DION (or some other ISP)

This is an important point with USEN. If there is a problem then you only have one number to call. If you go with OCN/NTT and there is an outage you call OCN - they blame NTT, so you call NTT - they blame OCN. 18 or so hours later the service returns to normal. This little story has happened to me around 8 times over the last 2 years all on business plans. Actually I have had the same problem with OCN, Plala and Interlink :-(

As for the router, the box that converts the actual fiber into a network
connection is going to belong to the fiber provider anyway. I have yet to
see a fiber router that one can purchase and, even if you could, you would
have one heck of a time hooking it up to the glass fiber ;-).

I have only used the USEN business plans and can attest to the fact that you get a media converter and that is all. Actually if you plug any DHCP client into the converter you will be assigned an IP from your own block.
