Well, I guess then you have these choices:
 1. Build a 2.6.16 kernel, from
    http://packages.debian.org/unstable/devel/linux-source-2.6.16
 2. Go back and use the ubuntu supplied binary kernel (2.6.15).
 3. Install the ubuntu 2.6.15 source and build the 0.4.5 version with it.

There is no package of the old version in debian.

I can't help you with that.

Check your installed kernels:
# dpkg -l 'linux-image-*' |grep ^ii
Then for each installed kernel, check the package contents:
# dpkg -L linux-image-with-the-exact-version
Then verify if the files listed under /lib/modules are still available.
If you have deleted them, reinstall the kernel:
# apt-get install --reinstall linux-image-2.6.15-whatever
This can save your ass if you deleted the modules.

Well, if you don't know how to configure a kernel, then let us forget
that. You could still build a kernel from source with the default config.
